Position Overview:
The Recovery Navigator plays a pivotal role within the Hub and Spoke Health Home model, dedicated to assisting individuals facing significant challenges related to substance use and co-occurring mental health disorders. This position involves collaborating closely with the Hub and Spoke Team to create tailored comprehensive care plans, conduct monthly recovery assessments, and ensure the care plans are updated regularly.

Key Responsibilities:

  1. Engage individuals using a strength-based, trauma-informed, and culturally sensitive approach, emphasizing the importance of connecting them to necessary care and services.
  2. Demonstrate a commitment to supporting individuals with substance use and mental health challenges.
  3. Work in tandem with the Lead Recovery Navigator and the Hub Team, including Certified Peer Specialists and Wellness Coaches, to effectively serve program participants.
  4. Provide direct support to approximately 12 individuals each week, ensuring their needs are met.
  5. Conduct a Recovery Life Story Tool and Assessment (RLST/PRP) in the individual's own words, focusing on their strengths and goals, with updates every 90 days.
  6. Deliver comprehensive care addressing various needs, including medical, housing, legal, financial, and social aspects.
  7. Facilitate monthly monitoring and team meetings in collaboration with the individual's care team.
  8. Coordinate with service providers to ensure the delivery of necessary services, maintaining regular communication with care team members.
  9. Assist individuals in building a network of informal and formal support systems based on their preferences.
  10. Provide transportation and accompany individuals to appointments, advocating for their identified needs.
  11. Support crisis intervention efforts and develop strategies for immediate needs, including relapse prevention plans.
  12. Adhere to all organizational policies and procedures, promoting a collaborative team approach to meet the diverse needs of individuals.
  13. Establish connections with community providers to foster collaborative relationships that benefit program participants.
  14. Document all interactions and activities related to individual care in the electronic medical record promptly.
  15. Complete all required training and uphold a positive, strength-based culture within the organization.
  16. Maintain confidentiality in all communications and adhere to established policies and procedures.
  17. Perform additional duties as assigned.

Qualifications:

  • High school diploma or GED required; a Bachelor's degree in Social Work, Psychology, Nursing, or a related field is preferred.
  • Experience working with individuals facing substance use challenges.
  • Proficiency in computer applications, particularly Windows and Microsoft Word.
  • Ability to demonstrate cultural competence and respect for diverse backgrounds.
  • Valid driver's license and reliable transportation.
  • Meet all employment requirements, including background checks and screenings.

Physical Demands:
This role may require extended periods of sitting and computer work, as well as travel within the community.

Work Environment:
Work is conducted in both office and community settings, requiring flexibility to meet the needs of individuals served.
