Job Description

Job Summary:

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Jewish Family Services Inc. As a Care Coordinator, you will play a vital role in providing rehabilitative and recovery services to individuals enrolled in our Comprehensive Community Services (CCS) program.

Key Responsibilities:

  • Conduct assessments of CCS consumers to identify recovery goals and develop customized recovery plans.
  • Meet with consumers in their homes or out in the community to monitor and support their progress towards recovery goals.
  • Work collaboratively with a dedicated team of providers to decide which programs and supports will assist consumers in reaching their recovery goals.
  • Provide case management services to adults with behavioral health, mental health, and/or addiction issues.
  • Develop and maintain relationships with consumers, agency staff, and other community professionals to ensure effective communication and coordination of services.

Requirements:

  • Four-year Bachelor's degree in a Human Services field (e.g. Social Welfare, Psychology, etc.)
  • Minimum of one (1) year of paid formal (non-family setting) experience working with adults providing Case Management or related services
  • Experience in behavioral health, mental health, and/or addiction is preferred
  • Experience in various case management systems (TCM, CSP, Wraparound Milwaukee, My Choice Family Care, etc.) is preferred
  • Excellent interpersonal, organizational, critical thinking, problem-solving, creative, and independent thinking skills
  • Ability to work independently with minimal supervision
  • Excellent verbal and written communication skills
  • Excellent customer service skills
  • Ability to maintain confidentiality and respect the privacy and dignity of consumers
  • Familiarity with Electronic Health Record (EHR) management systems
  • Proficient in computer use, Microsoft Office applications
