About the Role

We are seeking a highly skilled and compassionate Social Services Designee to join our team at Avantara Lake Zurich. As a Social Services Designee, you will play a vital role in ensuring our residents receive the highest standard of care and support during their stay with us.

Key Responsibilities
  • Assist residents in preparing for their future by coordinating discharge planning and participating in resident care conferences.
  • Ensure residents' social and emotional needs are met by providing personalized support and care.
  • Collaborate with the care team to develop and implement individualized care plans that meet the unique needs of each resident.
  • Provide education and support to residents and their families on various topics, including healthcare, wellness, and community resources.
  • Conduct regular assessments to identify residents' needs and develop strategies to address them.
Requirements
  • High school diploma or equivalent required; bachelor's degree in Social Work or a related field preferred.
  • At least two years of experience in a Social Worker role, preferably in a healthcare setting.
  • Prior experience as a Social Services Designee in a Skilled Nursing Facility is an asset.
