Veteran Care Coordinator

At Simple Steps LLC, we are seeking a highly motivated and compassionate Community Support Specialist to join our team. As a Community Support Specialist, you will play a vital role in providing high-quality home and community services to our Veterans with neurological disorders, physical disabilities, and traumatic brain injuries.
Key Responsibilities:- Deliver high-quality home and community services to our Veterans
- Support and carry out quality client care plans
- Orient and educate clients and their families about healthcare plan services and procedures
- Support accurate and complete documentation
- Supply timely data for generating accurate invoices
- Must be able to travel to current or prospective clients' homes
- Must be able and willing to transport client(s) during client visits
- Must have reliable transportation, a valid driver's license, and agency-required auto liability insurance (100k/300k Bodily Injury)
- Must be able to communicate with healthcare professionals, clients, families, and the general public through verbal and written communication
- Must have access to a private personal laptop or desktop computer for HIPAA compliance
- Mileage and Expense Reimbursement provided
- Flexible Staff Scheduling
