Job Overview
The Community Support Specialist plays a vital role in delivering home-based assistance, addressing social and emotional needs, acting as an advocate for patients, and managing the processes of admissions, discharges, and bereavement support for individuals and their families within the service area.
Key Responsibilities
1. Conducts evaluations or collaborates with the attending physician to assess the psycho-social requirements of patients and the capabilities of home caregivers in creating effective care plans.
2. Aids patients and caregivers in recognizing and accessing community resources and services.
3. Establishes and nurtures professional relationships with local agencies and organizations.
4. Exhibits the necessary expertise and skills to deliver care that is appropriate to the age, developmental stage, spiritual, and cultural needs of the patients served.
5. Accurately documents patient information using the designated electronic systems.
6. Ensures patient care adheres to the standards set by regulatory bodies to achieve the best possible outcomes.
7. Facilitates the resolution of concerns raised by patients and/or their families.
8. Manages timely and efficient admissions and discharges.
9. Responds promptly to the psycho-social needs of patients.
10. Demonstrates compassion and effective negotiation skills when evaluating transitional care requirements.
11. Provides supportive counseling to help patients and families adjust to illness, disability, and loss.
12. Prepares thoroughly for case discussions by documenting relevant information and engaging with the appropriate staff in a timely manner.
13. Identifies the need for further evaluation or services from other team members, consultants, or community resources, and initiates necessary referrals.
Qualifications
Required Education:
Bachelor's Degree from an accredited institution.
Preferred Education:
Bachelor's Degree in Social Work or a related human services field such as Sociology, Psychology, Gerontology, Special Education, or Rehabilitation Counseling from an accredited institution, or a Master's Degree in Social Work from an accredited institution.
Required Experience:
A minimum of two (2) years of experience in a healthcare environment utilizing an interdisciplinary approach, along with three (3) years of experience in casework or counseling within a healthcare or social service system. Proven skills in family counseling, group psychosocial assessment, crisis intervention, and familiarity with community resources are essential. Ability to collaborate effectively as part of an interdisciplinary team is crucial.
Demonstrated personal maturity, emotional resilience, and physical stamina are necessary to manage the challenges associated with working with patients facing end-of-life issues, grieving families, and other team members experiencing stress. Proficiency in computer systems is required.
