Job Summary

Key Responsibilities
  • Provide guidance and support to clients, including developing individualized service plans and performing psycho-social assessments.
  • Assist clients in identifying and accessing available resources, including federal, state, local, and private services.
  • Educate clients on clinical and other relevant procedures, and coordinate client care and community resources.
  • Develop and maintain relationships with clients, families, and community partners to ensure effective service delivery.
  • Document and maintain accurate records of client interactions and service delivery.
Requirements
  • Experience and knowledge of family services principles, practices, and ethics.
  • Ability to develop and implement treatment plans to deliver services and locate and secure resources.
  • Knowledge of infant and child development, parent-child dynamics, and child abuse, neglect, and trauma impact.
  • Strong verbal and communication skills, with ability to interview and gather data effectively.
  • Ability to work effectively as a team member and handle multiple tasks and responsibilities.
  • Computer skills, including web-based case management documentation.
