About the Position

HopeHealth is seeking an experienced Clinical Social Worker to join its affiliate HopeHealth Hospice & Palliative Care. This full-time position offers a competitive salary of $65,000 and benefits package.

Job Responsibilities
  • Develop and implement individualized social work plans for patients in their residences
  • Collaborate with interdisciplinary teams to identify and address significant social and emotional factors affecting patient care
  • Provide accurate and comprehensive documentation in electronic medical records
  • Travel to patient homes in service areas as needed
Requirements
  • Licensed LCSW/LICSW
  • Relevant clinical experience in healthcare settings
  • Medical social work background required; hospice experience preferred
  • Clinical experience working with family systems
