Patient Connect Nurse Practitioner

Join Optum Home & Community Care, a leading healthcare organization, as a Patient Connect Nurse Practitioner. In this unique role, you will play a vital part in ensuring a smooth transition from rehabilitation to home for patients who have recently been hospitalized.
Key Responsibilities:
- Perform comprehensive assessments and document findings in a concise and compliant manner.
- Ensure accurate and timely documentation of diagnoses, medications, and quality elements.
- Communicate effectively with patients, families, and healthcare teams to coordinate care.
- Travel between care sites to provide care to patients in various settings.
Requirements:
- Certified Nurse Practitioner (NP) or Certified Physician Assistant (PA) through a national board.
- Graduate of an accredited masters degree in Nursing (MSN) program or accredited Physician Assistant degree program.
- Active and unrestricted license in the state of practice.
- Current active DEA licensure/prescriptive authority or ability to obtain post-hire.
- Access to reliable transportation and ability to lift a 30-pound bag.
Preferred Qualifications:
- 1+ years of hands-on post-grad experience in Long Term Care.
- Understanding of Geriatrics and Chronic Illness.
- Proficient computer skills, including electronic medical records.
