Job Summary:

Optum is seeking a skilled and compassionate Advanced Practice Clinician to join our team as a Patient Connect Specialist. In this role, you will work with patients who have recently been hospitalized and are in a skilled rehab facility, providing comprehensive assessments and documentation to ensure a smooth transition from rehab to home and prevent readmission.

Key Responsibilities:
  • Perform comprehensive assessments and document findings in a concise and compliant manner.
  • Ensure accurate and appropriate documentation of diagnoses and quality elements.
  • Communicate with patients and team members regarding medical concerns.
  • Participate in quality initiatives and maintain positive customer relationships.

Requirements:
  • Certified Nurse Practitioner or Physician Assistant through a national board.
  • Graduate of an accredited master's degree in Nursing or Physician Assistant degree program.
  • Active and unrestricted license in the state of practice.
  • Ability to lift a 30-pound bag and navigate stairs and various dwelling conditions.

Preferred Qualifications:
  • 1+ years of hands-on post-grad experience in Long Term Care.
  • Understanding of Geriatrics and Chronic Illness.
  • Proficient computer skills and ability to document medical information.
