Job Summary

Optum Home and Community Care is seeking a skilled and compassionate Advanced Practice Clinician (APC) to join our team. As an APC, you will play a critical role in ensuring a smooth transition from rehab to home for patients who have recently been hospitalized. Your expertise and experience will be invaluable in providing guidance and support to patients and their families during this challenging time.

Key Responsibilities
  • Conduct comprehensive assessments and document findings in a concise and compliant manner.
  • Ensure accurate and timely documentation of diagnoses, medications, and treatment plans.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in quality initiatives and contribute to the development of new and improved methods and approaches to care.
  • Travel between care sites as needed to provide care and support to patients.
Requirements
  • Certified Nurse Practitioner (NP) or Physician Assistant (PA) through a national board.
  • Graduate of an accredited master's degree in Nursing (MSN) program or Physician Assistant degree program.
  • Active and unrestricted license in the state of practice.
  • Current active DEA licensure/prescriptive authority or ability to obtain post-hire.
  • Access to reliable transportation and ability to navigate stairs and varying dwelling conditions.
  • Availability to work 24 hours per month, with flexibility to work off-hours and weekends.
Preferred Qualifications
  • 1+ years of hands-on post-grad experience in Long Term Care.
  • Understanding of Geriatrics and Chronic Illness.
  • Understanding of Advanced Illness and end of life discussions.
  • Proficient computer skills and ability to document medical information.
  • Ability to develop and maintain positive customer relationships.
