Job Summary

The Illumination Foundation is seeking a highly skilled and compassionate Medical Coordination Assistant to join our team. As a key member of our healthcare team, you will play a vital role in ensuring the optimal facilitation of medical care for homeless clients discharged from hospitals or other agencies contracted with our Recuperative Care Centers.

Key Responsibilities
  • Initial Medical Assessments: Conduct thorough head-to-toe assessments, check vital signs, provide patient education, determine insurance status, and schedule medical follow-up appointments with clients' doctors.
  • Client Information Management: Input and update client information and medical notes in our electronic medical record (EMR) system.
  • Client Support and Education: Provide patient education on medical treatment, diet, and personal healthcare procedures to promote health literacy.
  • Collaboration and Advocacy: Work closely with Medical Coordinators and Healthcare Providers to ensure best patient outcomes and serve as an advocate for clients to link them to medical resources and MediCare or Medi-Cal as needed.
Requirements
  • Education: High School Diploma or equivalent.
  • Certifications: Current and non-restricted Medical Assistant (MA) Certificate or Emergency Medical Technician (EMT) license.
  • Computer Skills: Basic computer skills, including the ability to send and receive emails and summarize data in spreadsheets.
Preferred Qualifications
  • Experience: 2 years of relevant experience.
  • Language Skills: Bilingual in English and Spanish.
  • Software Proficiency: Proficiency in Microsoft Office Suite (Outlook, Word, Excel, Teams).
  • Driver's License: Valid CA Driver's License.
