As a Licensed Practical Nurse (LPN) in our Home Health department, you will play a vital role in providing high-quality care to patients in their homes. This is a full-time position that requires strong clinical skills, excellent communication abilities, and a passion for delivering patient-centered care.

  Responsibilities:
  The duties of this role include implementing and evaluating patient care plans, administering medications and treatments as prescribed, performing focused systems and wound assessments, and documenting patient care in the health record according to department standards. You will also be responsible for communicating effectively with patients, families, and healthcare providers to ensure seamless care coordination.

  Requirements:
  To be considered for this position, you must have a graduate degree from an accredited State Board of Nursing LPN program, current licensure as an LPN in the state where you will be practicing, and Basic Life Support (BLS) certification. You should also possess excellent communication skills and be able to work effectively with individuals diagnosed with mental and behavioral disorders. A valid driver's license and proof of auto insurance are also required.

  Compensation:
  This position offers a competitive hourly rate ranging from $23.50 to $38.06, dependent on qualifications and experience. You may also be eligible for a hiring bonus and relocation assistance.
