We are seeking a compassionate and skilled LPN to join our home health team. As an LPN Home Health Specialist, you will provide direct nursing care to patients in their homes, working closely with physicians and other healthcare providers.

About the Role

Under the supervision of an RN, you will implement and evaluate patient care plans, participate in patient-specific care planning, and provide direct patient care, including administering medications and treatments as prescribed.

You will have the opportunity to work independently in a home setting, communicating effectively with individuals diagnosed with mental and behavioral disorders. Your documentation skills will be essential in maintaining accurate records of patient care in accordance with department standards.

Requirements
  • A graduate of an accredited State Board of Nursing LPN program
  • Demonstrated knowledge of age-specific principles of growth and development and life stages to meet each patient's needs
  • Current licensure as an LPN in the state where you will practice
  • Current BLS (Basic Life Support) certification required at date of hire
  • A valid driver's license and proof of auto insurance
What We Offer

We offer a competitive salary range of $32,116 - $47,644 per year, depending on education, training, work experience, and collective bargaining agreement terms. In addition, we offer a hiring bonus and relocation assistance.
