Job Summary

This is a full-time position responsible for providing high-quality nursing care to patients in their homes. The successful candidate will possess strong clinical skills, excellent communication abilities, and a compassionate approach to patient care. As a key member of our interdisciplinary team, you will work collaboratively with other healthcare professionals to develop and implement individualized care plans tailored to each patient's unique needs.

  • Provides comprehensive nursing care to patients in their homes, including pain management, symptom control, and education on disease management and self-care.
  • Collaborates with the interdisciplinary team to assess patient needs, develop care plans, and coordinate services to meet those needs.
  • Maintains accurate and timely documentation of patient care activities, including progress notes, medication lists, and vital signs.
  • Communicates effectively with patients, families, and caregivers to ensure seamless care transitions and address concerns or questions in a timely manner.
Requirements

To be considered for this role, applicants must possess:

  • A current RN license in the state of Washington.
  • A minimum of one year of experience in an acute-care setting, home health care, community health, or public health within the last 24 months.
  • Valid driver's license and reliable transportation.
  • Proof of automobile insurance and a satisfactory motor vehicle record (MVR).
  • Current CPR certification and ability to pass a criminal background check and national sex offender clearance.
