At PeaceHealth, we seek a dedicated RN Clinical Care Manager to join our team in an ambulatory setting. Salary: $55,000 - $75,000 per year
The ideal candidate will have a minimum of 3 years of clinical nursing experience in an ambulatory or clinic setting and possess strong technical skills. A Bachelor's Degree in Nursing is preferred, along with experience in care management or utilization management/quality management.
Job Description:
- Overses and evaluates coordination of integrated population management across selected populations to improve clinical and financial outcomes.
- Develops care management programs addressing a wide scope of health care needs and conditions in partnership with physicians, healthcare professionals, and patients.
- Risk-stratifies patients according to protocols, assesses patient data, and works within an interdisciplinary care team to assess patients' physical, cognitive, emotional, and behavioral status as well as social and financial support systems.
Responsibilities may include:
- Delegates and oversees the care management of lower-risk patients as well as routine chronic disease population management tasks to assigned caregivers.
- Tracks, evaluates, and provides oversight for transitions of care, synchronizing 'warm handoffs' between the clinic and hospitals, emergency departments, or care facilities.
What You Bring:
- A minimum of 3 years of clinical nursing experience in an ambulatory or clinic setting.
- Strong technical skills, including proficiency in applying highly technical principles, concepts, and techniques.
- Experience in care management or utilization management/quality management is preferred.
Benefits:
