Typical Schedule:
Primarily Monday to Friday during daytime hours. Flexibility is required, including the possibility of a hybrid work arrangement (remote and in-person). Participation in an emergency call rotation is expected, with on-call duties for one week every six weeks.

Position Overview:
The role of the nurse is to enhance the health and well-being of adults with intellectual and developmental disabilities in both group home and apartment environments. The nurse acts as a health advocate, collaborating with individuals and the interdisciplinary team. This position supports direct care staff in ensuring compliance with state and federal regulations pertaining to health services.


Core Responsibilities:
1. Nursing Assessments
  • Monitor the health status of assigned individuals.
  • Communicate any changes in health conditions to relevant team members.
  • Triage health-related concerns in a timely manner.
  • Coordinate necessary medical services and integrate recommendations into individuals' Support Plans.
  • Develop and implement health care plans for ICF/DD programs.

2. Health Education and Wellness Promotion
  • Educate and counsel individuals, their families, and staff on health and wellness topics, fostering self-responsibility and independence.

Respond promptly to calls on the emergency nurse cell phone as part of a rotating schedule. Advocate for awareness of the evolving needs of individuals with developmental disabilities within the community. Ensure adherence to health and safety standards, including HIPAA and PHI regulations.

  • Participate in the periodic review of Support Plans as a member of the interdisciplinary team.

Assist in training staff during medical orientation sessions and annual continuing education programs.

  • Engage in discussions regarding infection control and its implementation across the organization.
  • Enforce all health service policies and procedures in line with state and federal regulations.

Support the development of health-related protocols and procedures for individuals and households. Attend and contribute to mandatory staff meetings and training sessions.
Communicate effectively and collaborate with colleagues and administrative staff.
Provide backup nursing support when the assigned nurse is unavailable.
Ensure all paperwork is processed accurately and promptly, with a focus on detail.

Essential Qualifications:
  • Current licensure as an LPN or RN in good standing with the State of Minnesota.
  • Proven experience in managing health services for individuals with intellectual disabilities and related conditions.
  • Valid driver's license and a driving record that meets the requirements of the organization’s insurance carrier.

Must pass a MN DHS Background study and provide evidence of a negative Tuberculosis test result.

  • Strong written and verbal communication skills, including proficiency in English and familiarity with Microsoft Office applications (especially Outlook and Excel) and SharePoint.

Demonstrated organizational skills, with the ability to manage multiple projects simultaneously, strong time management abilities, attention to detail, and autonomy based on individual health needs. A commitment to confidentiality and exceptional customer service is essential.

Physical Requirements:
Frequent use of a telephone and repetitive motion activities, including typing on a computer keyboard. Ability to work for extended periods at a desk and computer. Must be capable of performing nursing assessments, evaluations, and skilled nursing tasks, including bending and lifting.
