Care Manager for Assisted Living and Reminiscence Neighborhoods

As a Care Manager at Sunrise Senior Living, you will play a vital role in providing exceptional care and services to our residents and their families. Our mission is to champion quality of life for all seniors, and we are committed to delivering high-quality care with a personal touch.
Responsibilities- Build meaningful relationships with a specified number of seniors and their families, providing assistance with activities of daily living, attending to individual care needs, and getting to know their unique preferences and personalities.
- Notify management of changes in condition and recommend adjustments in the level of care and service.
- Assist residents in life skills and other life-enriching activities as indicated on their individual profile.
- Participate in the development of Individualized Service Plans (ISPs) and monthly updates.
- Be responsible for your designated group of residents during the shift, knowing where they are and physically checking on them throughout the shift.
- Partner with the community team to ensure compliance with OSHA requirements and promotion of Risk Management programs and policies; adhere to safety rules and regulations.
- Respond to dining needs of residents and guests while maintaining Sunrise hospitality and service standards.
To be successful in this role, you should have:
- Dedication to and passion for serving seniors with excellent customer service skills.
- A positive attitude, flexibility to perform various duties in service to residents, and ability to work in a team environment.
- A High School diploma or GED accepted, with potential requirement per state regulations.
- In states where applicable, must maintain certifications.
- Must be at least 18 years old.
- Previous experience working with seniors is preferred.
- Ability to make choices, decisions, and act in the resident's best interest.
- Possess written and verbal skills for effective communication and a level of understanding.
- Competent in organizational and time management skills.
- Demonstrate good judgment, problem-solving, and decision-making skills.
