Job Summary
The Geriatric Care Manager - LPN is responsible for providing comprehensive care and support to residents, including administering medication, conducting service and health updates, and maintaining accurate records. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment. The ideal candidate will be a licensed practical nurse (LPN) with experience working in geriatric care settings.

Responsibilities
- Administer medications and treatments as prescribed by physicians
- Conduct service and health updates, documenting resident progress and any changes in condition
- Maintain accurate and up-to-date records, including medication administration records and service plans
- Collaborate with other healthcare professionals to ensure seamless care delivery
- Develop and implement individualized care plans, taking into account resident needs and preferences

Requirements
- Current licensure as a Licensed Practical Nurse (LPN)
- Experience working in geriatric care settings, preferably with seniors or adults with disabilities
- Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare teams
- Ability to maintain confidentiality and handle sensitive information discreetly
- Basic computer skills and familiarity with electronic medical records systems
