Job Responsibilities:

  • The Patient Care Coordinator is responsible for overseeing the coordination of clinical services for patients transitioning from hospital to home care.
  • This role involves promoting, educating, and facilitating services during the patient's transfer to home healthcare or hospice care.

Qualifications:

  • A Bachelor’s degree in nursing is preferred.
  • Graduation from a CCNE or NLN accredited nursing program is strongly preferred.
  • A minimum of 2 years of experience in home health or hospice care is preferred.
  • Familiarity with community resources is advantageous.

Licensure Requirements:

  • A valid Registered Nurse license in the State of South Dakota or eligibility for multi-state licensure is required.
  • Integrated Care Management Model Certification is preferred.
  • A valid driver's license and insurability under Avera Health's automobile liability insurance are required.

Work Schedule:

  • Standard hours are from 8:00 am to 4:30 pm as needed; PRN.

Compensation:

  • The hourly wage for this position is competitive, with a range starting at $44.25 per hour, based on experience.
