Position Overview:
- The Patient Care Coordinator is responsible for managing the coordination of clinical services for patients transitioning from hospital to home care.
- This role involves promoting, educating, and facilitating services during patient transfers to home healthcare or hospice care.
Qualifications:
- A Bachelor's degree in nursing is preferred.
- Graduation from a CCNE or NLN accredited nursing program is strongly preferred.
- A minimum of 2 years of experience in home health or hospice care is preferred.
- Familiarity with community resources is advantageous.
Licensure and Certification:
- Registered Nurse license in the State of South Dakota or meeting multi-state licensure requirements is mandatory.
- Integrated Care Management Model Certification is preferred.
- A valid driver's license and insurability under Avera Health's automobile liability insurance are required.
Work Schedule:
- Hours are typically from 8:00 am to 4:30 pm as needed; PRN.
Compensation:
- The hourly wage for this position is competitive, with actual rates based on experience.
- Hourly rate starts at $44.25.
