Key Responsibilities:

  • The Patient Care Coordinator is responsible for overseeing the integration of clinical services for patients transitioning from hospital to home or other care settings.
  • This role involves promoting, educating, and coordinating services during patient transfers to home healthcare or hospice care.

Qualifications:

  • A Bachelor's degree in nursing is preferred.
  • Graduation from a CCNE or NLN accredited nursing program is strongly preferred.
  • A minimum of 2 years of experience in home health or hospice care is preferred.
  • Familiarity with community resources is advantageous.

Licensure Requirements:

  • Registered Nurse license in the State of South Dakota or eligibility for multi-state licensure is required.
  • Integrated Care Management Model Certification is preferred.
  • A valid driver's license and insurability under Avera Health's automobile liability insurance are necessary.

Work Schedule:

  • Shifts typically run from 8:00 am to 4:30 pm as needed; PRN availability is required.

Compensation:

  • The hourly wage for this position is competitive, with actual rates determined by experience.
  • Hourly rate starting at $44.25.
