Care Management Director

We are seeking a seasoned Care Management Director to lead our existing Care Management Programs and future innovative population health initiatives. This role will be responsible for designing and implementing models of care for D-SNP members and overseeing other lines of business.
Key Responsibilities:
- Partner with senior leadership to develop and execute annual strategic plans, ensuring appropriate resource allocation and budgeting for the care management department.
- Act as a change agent to improve performance results, providing organizational effectiveness leadership to achieve department goals.
- Lead a team focused on high-quality patient care and other high-member-touch programs.
- Establish strong partnerships across the organization, including People & Engagement, Finance, Enterprise Project Management Office, and IT.
- Collaborate with multiple business areas to address cross-functional organizational problems and ensure seamless program implementations.
- Develop and maintain a training and learning development plan, ensuring ongoing business process management and continuous process improvement.
- Drive strategic outcomes of Care Management programs, evaluating data from multiple sources and designing programs in conjunction with clinical leaders.
- Ensure key Care Management metrics related to compliance and contractual obligations are met, maintaining audit readiness and subject matter expertise in regulatory and process areas.
- Maintain staffing and development of the Care Management team, working cross-functionally to start new initiatives and lead their operationalization.
Requirements:
- Current Registered Nurse (RN) license in the state of California.
- Bachelor's Degree.
- Master's Degree Preferred.
- Minimum of 3 years' experience successfully managing people and leading teams.
- Minimum of 3 years' progressive leadership experience in MediCal/Medicare care management, with model of care experience.
- Minimum of 7 years' combined experience managing projects, programs, complex change initiatives, and/or CMS and DHCS audits and regulatory compliance.
- Minimum of 7 years' experience in developing and executing strategic business plans and budgets with a track record of achieving results.
