Job Title: Service Coordinator

Location: Washington, PA and Allegheny County Area

We are seeking a highly motivated and organized Service Coordinator to join our team at A Bridge to Independence. The successful candidate will be responsible for providing service coordination activities for consumer caseloads, ensuring that individuals receive the necessary support to maintain their independence.

Key Responsibilities:

  • Complete service coordination activities for consumer caseloads
  • Perform related duties as assigned, incorporating the independent living philosophy and consumer choice/control within the service coordination operational structure
  • Conduct scheduled and as-needed phone calls to and from consumers
  • Conduct home visits in consumer's homes
  • Complete intakes and annual re-certifications/re-assessments as required
  • Document all service coordination activities as required

Requirements:

  • Bachelor's degree, including or supplemented by at least 12 college-level credits in sociology, social welfare, social work, psychology, gerontology, or another behavioral science
  • Combination of experience and training that adds up to 4 years of experience, and education that includes at least 12 semester hours of college-level courses in sociology, social work, social welfare, psychology, or gerontology, or other social science
  • Required training, including OLTL Service Coordinator training
  • Ability to read and interpret documents, write routine reports, and speak before groups
