Job Summary

Element Care is seeking a highly skilled and compassionate Nurse Practitioner to join our interdisciplinary team. As a key member of our healthcare team, you will provide comprehensive primary care to our participants, working collaboratively with our Medical Doctors to ensure the highest level of care.

Key Responsibilities
  • Initial and Quarterly Examinations: Conduct thorough examinations of new participants and quarterly examinations for the first year of enrollment.
  • Evaluations and Follow-ups: Evaluate participants for episodic visits, acute illnesses, and follow-ups of hospitalizations.
  • Nursing Home and Home Visits: Make nursing home visits in accordance with state requirements and as needed, as well as home visits for acutely or chronically ill participants who are homebound.
  • After Hours On-Call Coverage: Participate in after-hours on-call coverage to ensure seamless care for our participants.
  • Healthcare Teaching: Provide education to patients, families, caregivers, and other team members on health care topics.
  • Documentation and Compliance: Maintain accurate and timely documentation, adhering to established standards and guidelines.
  • Fiscal Responsibility: Ensure duties are accomplished in a fiscally responsible manner, adhering to budgetary constraints.
  • Client Interactions: Interact with clients in a manner that adheres to the highest standards of care.
  • Travel and Other Duties: Perform frequent local travel and other duties as assigned.
Requirements
  • Education: Master of Science Degree in Nursing (MSN).
  • Certification: Current Board Certification as a Nurse Practitioner in the Commonwealth of Massachusetts.
  • Licensure: Massachusetts and DEA Prescription Privileges.
  • Credentials: Credentialed for the Element Care affiliate Long Term Care Facilities.
  • CPR Certification: Current CPR Certification.
  • Experience: Minimum of one (1) year working with a geriatric population.
  • Communication and Leadership: Ability to convey information clearly and succinctly with all employees, lead independently, and work well with others.
  • Attention to Detail: Ability to pay attention to detail, be a self-starter, and complete tasks on time.
  • Problem-Solving: Ability to quickly recognize situations/problems before they become acute, and implement effective solutions and understand consequences.
