This position involves collaboration with a multidisciplinary team to provide essential health information within a framework of supportive and restorative care.
The LPN will be responsible for gathering data regarding the patient's health status, in partnership with other healthcare professionals, to identify health objectives.
Our LPNs ensure that care for patients and their families aligns with our organization’s core values and are well-versed in the primary nursing model.
Work Schedule:Part-Time / 32 Hours per week / Day Shift / 9:30 AM - 6:00 PM, 4 days a week. Monday through Friday, no holidays.
Experience requirements vary based on unit needs. Candidates must have graduated from an accredited practical nursing program.
The LPN will apply the necessary skills to execute the medical/nursing care plan, administer medications, and perform procedures essential for patient care under the supervision of the Registered Nurse.
Direct patient care will be provided within the primary nursing model to ensure continuity and safety for all patients in their care, while upholding the patient's rights and dignity at all times.
Key responsibilities include:
- Performing technical aspects of patient care (medications, treatments, etc.) safely, in accordance with established policies and the Nurse Practice Act.
- Documenting pain management and communicating any changes in the patient's condition to the Registered Nurse promptly.
- Base all nursing interventions on pathophysiological and psychosocial rationale while delivering care in a fiscally responsible manner.
