Job Summary

Hackensack Meridian Health is seeking a highly skilled Advanced Practice Nurse to join our Transitional Care team in the COPD/Pulmonary population. This is a full-time position that requires leadership and progressive leadership skills, as well as experience in transitional care management.

Responsibilities
  • Patient Assessment and Care Planning

Obtains a thorough patient history, including determination of their health status and primary care service needs, analysis of primary symptoms, and assessment of patient compliance with historical primary care treatment plans. Performs an appropriate physical examination and recognizes normal and abnormal findings. Records findings of the physical exam in a clear and concise manner using the appropriate format.

Communication and Collaboration

Communicates with assigned Primary Care physicians regarding pertinent patient information, including actual or potential clinical problems. Collaborates with physicians, family members, social workers, community contacts, and other members of the healthcare team in patient care planning.

Education and Plan of Care

Utilizes assessment skills and collaborates with other disciplines to develop a comprehensive education plan to meet patient/family needs. Develops a comprehensive plan of care that meets patient needs, incorporating diagnosis, interventions, and expected outcomes. Communicates plans of care with the patient and family.

Leadership and Professional Development

Understands his/her role as a professional and conducts him/herself in accordance with the same. Seeks out other professionals to network and create positive alliances. Is committed to a positive, meaningful interaction with patients. Oversees Community Outreach workers and conducts weekly meetings to discuss patient progress, goals, and issues/concerns.

Other Duties and Projects

Performs other duties and/or projects as assigned. Adheres to Hackensack Meridian Health Organizational competencies and standards of behavior.

Qualifications
  • Education and Experience

Master's Degree in Nursing with Advanced Nurse Practitioner credentials from an accredited Nurse Practitioner Program required. Demonstrates effective communication and leadership skills, including teaching principles, organizational, and presentation skills. Computer skills with a working knowledge of electronic medical records required.

Licenses and Certifications

Advance Practice Nurse License. AHA Basic Health Care Life Support HCP Certification. C.D.S. (Controlled Dangerous Substance) registration is required for APNs who will be prescribing controlled dangerous substances. D.E.A. (Drug Enforcement Agency) certification is required for APNs who will be prescribing controlled dangerous substances.
