Home Health LPN Job Description

Millennium Home Care, a trusted leader in the home health industry, is seeking a dynamic and experienced Licensed Practical Nurse (LPN) to join our team on a full-time basis.

Responsibilities:
  • Provide exceptional nursing care to patients in the comfort and familiarity of their own homes.
  • Administer medications and treatments as prescribed by physicians, ensuring the highest standards of patient safety and well-being.
  • Monitor patient vital signs, assess health status, and communicate any changes or concerns to the healthcare team.
  • Collaborate with interdisciplinary team members to develop personalized care plans tailored to each patient's unique needs.
  • Educate patients and their families on effective healthcare management and disease prevention strategies.
Requirements:
  • Valid and active LPN license in Florida, demonstrating your commitment to excellence in nursing practice.
  • Minimum of 1 year of experience in home health or a related field, showcasing your expertise in providing quality care in a home setting.
  • Proficiency in nursing assessments and care planning, allowing you to deliver personalized and patient-centered care.
  • Excellent communication and interpersonal skills, enabling you to establish rapport and build trusting relationships with patients and their families.
  • Ability to work independently while also collaborating effectively within a multidisciplinary team.
  • Valid driver's license and reliable transportation to travel to patients' homes.
