POSITION SUMMARY

The Community Reintegration Specialist facilitates access to community resources, ensures continuity of care post-release, advocates for client needs, and serves as a liaison among various systems affecting individuals with mental health challenges, co-occurring substance use disorders, and chronic health conditions.

MINIMUM QUALIFICATIONS

  • A bachelor's degree in Mental Health or a closely related discipline.
  • At least one year of experience in the social services sector.
  • Experience in Case Management.
  • Preference will be given to candidates with experience in a correctional healthcare environment.
