Healthcare Liaison

The Engagement Specialist plays a vital role in ensuring clients receive timely and effective services. This position requires a proactive approach to learning about healthcare services, benefit assistance, and basic health screening processes. The ideal candidate will have a general knowledge of the healthcare system and be able to think outside the box to connect clients to the right care.
Key Responsibilities
• Ensure clients receive services in a timely and effective manner, identifying and removing barriers to access
• Facilitate communication between clients, family members, and the care team to ensure client satisfaction and quality of care
• Evaluate social determinants of health and link clients to resources such as housing supports, food, basic needs, financial assistance, and employment resources
• Intervene when appointments are missed to determine reason, barriers to access, and identify any desires for care to be adjusted
• Identify and link clients to proper services that address those who have limited English proficiency, are deaf or hard of hearing, or who require age-related or cognitive accommodations
• Ensure continuity of care between internal and external providers, support transitions or adjustments to care, and provide linkages to community resources
• Advocate with providers and payors for proper healthcare
• Determine creative solutions to barriers to care directly or through facilitating connections to internal or external resources for issues such as transportation, housing, and wellness
• Become familiar with the network of medical providers in the area and how to apply and access benefits
• Track appointments, medication, and other wellness needs for clients on caseload as needed
• Connect with the care team and help ensure clients do not fall through the cracks using creativity, teamwork, and a whole health approach
Requirements
• High School graduate or equivalent
• Minimum of 6 months experience in healthcare or customer service
• Valid driver's license, proof of auto insurance, and reliable transportation
• Preferred Education and Experience: Experience providing case management services, CMA or bachelor's degree in Psychology, Social Work, Public Health, or related field, and experience in integrated care services
