Care Manager RN – CW

At Tech Providers Inc., we are seeking a highly skilled Care Manager, RN – CW to join our team. As a key member of our care management team, you will play a critical role in ensuring the best possible outcomes for our members.
Key Responsibilities:- Registered Nurse in the State of DE: We require a Registered Nurse with a valid license in the state of Delaware.
- Intensive Case Management Experience: You should have at least 5 years of experience in intensive case management and discharge planning.
- Assessment and Service Planning: You will be responsible for conducting assessments, developing service plans, and care plans that meet the unique needs of our members.
- Collaboration and Communication: You will work closely with primary care physicians, occupational therapists, behavioral health professionals, and other providers to ensure seamless care coordination.
- Discharge Planning Expertise: You must have experience discharging members from facility settings and ensuring a smooth transition to community-based care.
- Flexibility and Adaptability: You will work flexible hours to meet the needs of our members, including evenings and weekends as required.
- Technical Skills: You should be proficient in PC-based word processing, database documentation, and electronic documentation systems.
- Reliable Transportation: You will need reliable transportation to travel within your assigned territory.
- Regulatory Compliance: You must be able to meet regulatory deadlines and ensure compliance with all relevant laws and regulations.
- Cultural Competency: You should have an understanding of the importance of cultural competency in addressing the needs of targeted populations.
