Job Title:

Home Health and Hospice Registered Nurse

Job Summary:

We are seeking a compassionate and skilled Registered Nurse to join our Home Health and Hospice team. As a Home Health and Hospice Nurse, you will provide high-quality patient care and support to patients and their families in the comfort of their own homes.

Responsibilities:
  • Educate patients, caregivers, and family members on nursing principles and techniques
  • Participate in the development and implementation of the Plan of Care
  • Provide patient, caregiver, or family counseling and referrals as needed
  • Initiate preventative and rehabilitative nursing procedures
  • Conduct OASIS assessments and report patient conditions to the physician
  • Maintain communication with the supervisor regarding daily visit schedules
Requirements:
  • Registered Nurse licensure in the State of Idaho
  • Current BLS certification; ACLS optional
  • At least one year of direct patient care experience
  • Working knowledge of nursing, healthcare safety, state and federal regulations, and regulatory processes
  • Excellent written and verbal communication skills in English

Work Environment:

Franklin County Medical Center operates 24 hours a day, 7 days a week. You will be required to work all shifts, weekends, and holidays to meet the needs of our patients.

Our Location:

Franklin County is surrounded by the beautiful Rocky Mountains, offering ample opportunities for outdoor recreation. We are located in southeast Idaho, within 30 minutes of Logan, Utah, and 1 hour from Pocatello, Idaho.
