Job Summary

We are seeking a skilled Occupational Therapist to join our team at Symbii Home Health and Hospice. As an Occupational Therapist, you will play a vital role in helping our patients achieve their goals and improve their quality of life.

Key Responsibilities
  • Conduct thorough assessments of patients' functional status and develop individualized plans of care.
  • Provide patient, caregiver, and family counseling to promote independence and well-being.
  • Collaborate with healthcare professionals to ensure comprehensive care and coordinate services.
  • Evaluate home environments for hazards and barriers to independent living, and identify equipment needs.
  • Develop and implement treatment programs to improve patients' functional abilities and quality of life.
Requirements
  • OT licensed in the State of Idaho.
  • Current CPR certification.
  • Valid driver's license and auto insurance.
  • Home health and hospice experience preferred.
