Job Summary:


The Speech-Language Pathologist PRN is responsible for implementing standards of care for speech language pathology services and ensuring adherence to all conditions in the Service Agreement.


Duties and Responsibilities:

Conducts evaluations, assesses home environments, and identifies equipment needs related to speech/language function.
Assumes responsibility for ongoing interdisciplinary assessment and development of individualized plans of care in partnership with patients, representatives, and caregivers;
Provides services ordered by physicians as indicated in plans of care;
Provides patient, caregiver, and family counseling;
Provides patient and caregiver education;
Prepares clinical notes;
Communicates with physicians responsible for home health plans of care and other healthcare practitioners related to current home health plans of care;
Identifies functional speech defects and establishes plans of care to improve patient function;
Administers speech therapy programs utilizing specialized therapeutic techniques and equipment;
Recommends communication devices/aids as indicated;
Participates in case conferences, in-services, and other meetings to ensure coordinated and comprehensive care;
Maintains clinical competency in speech language pathology practice and theory;
Identifies patient and family/caregiver needs for other services and refers as appropriate;
May supervise home health aides in procedures directly related to speech-language therapy plans of care;
Participates in HHA's quality assessment and performance improvement program and HHA-sponsored in-service training;
Performs other duties as required to facilitate delivery of speech language pathology services;
