Job Summary

Porch Light Health is seeking a dedicated and compassionate Patient Care Coordinator to join our team. As a key member of our outpatient treatment team, you will play a vital role in supporting our patients on their journey to recovery.

Responsibilities:

  • Provide exceptional patient care and support in a fast-paced medical environment
  • Assist with scheduling, collecting vital signs, and registering patients
  • Answer phone calls and direct patients to the appropriate staff member
  • Admit and register patients, ensuring accurate and timely documentation
  • Maintain accurate and up-to-date records, adhering to HIPAA and OSHA guidelines
  • Participate in building maintenance and cleaning, ensuring a safe and healthy environment for patients and staff
  • Communicate effectively with medical staff, patients, and families to provide excellent customer service
  • Demonstrate a high-level understanding of Medication Assisted Treatment (MAT), Harm Reduction, Low Barrier/High Access, and safety practices

Requirements:

  • Previous medical clinic and/or addictions treatment experience preferred
  • Basic computer skills and reliable transportation
  • Proper time management and reliability

Work Schedule:

  • 8 hour shift, 32 hours per week, Tues-Friday

Education and Experience:

  • High School Diploma or GED equivalent
  • Medical office experience required

License/Certification:

  • Driver's License required
