Job Summary

We're seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will be responsible for effectively utilizing hospital resources to develop care plans, conduct discharge planning, and perform utilization review. You will work closely with patients, families, and healthcare teams to ensure seamless transitions and optimal patient outcomes.

Responsibilities
  • Develop and implement care plans that meet individual patient needs.
  • Conduct discharge planning and coordinate post-acute care services.
  • Perform utilization review to ensure appropriate resource utilization.
  • Collaborate with healthcare teams to identify and address patient needs.
  • Provide education and support to patients and families.
Requirements
  • ADN or BSN degree required.
  • Current licensure applicable to professional affiliation required.
  • Certification by the National Association of Healthcare Professionals or InterQual is desirable.
  • Current New Mexico licensure as a Registered Nurse required.
  • Certification as CCM or ACM desired.
