About the Role

We are seeking a highly skilled Care Coordinator to join our team at PSG Global Solutions Careers. As a Care Coordinator, you will play a critical role in ensuring the smooth delivery of healthcare services to our clients.

Key Responsibilities
  • Home Health Assessments: Conduct thorough assessments of patients' health needs and develop personalized care plans to meet their requirements.
  • Member Outreach: Establish and maintain relationships with patients to encourage compliance with care plans and address any issues that may arise.
  • Communication and Education: Inform and educate patients about their healthcare programs, using supplied scripts as needed.
  • Data Entry and Record Keeping: Accurately complete records in our system by performing data entry and maintaining up-to-date patient information.
  • Program Promotion: Encourage patient participation in our programs, including arranging appointments and additional services as required.
  • Correspondence and Communication: Generate and send correspondence to patients manually, electronically, or telephonically, as needed.
  • Check-Ins and Progress Monitoring: Conduct regular check-ins with patients to review their care plan goals and progress.
  • Production Requirements: Meet established departmental production requirements to ensure the efficient delivery of healthcare services.
  • Support to Clinical Team: Provide non-clinical support to our clinical team, including generating, managing, and closing cases within our platform.
  • Customer Service and Inquiries: Respond to patient inquiries, analyze their needs, and provide support to resolve any issues.
  • System Issues and Notifications: Notify our help desk of any system issues and perform data entry to update patient information.
  • Research and Communication: Conduct research, obtain medical records, and communicate with patients and providers to determine their needs and address their concerns.
  • Claims and Communications: Serve as a contact for patients and providers regarding claims, including conducting research, obtaining medical records, and referring cases to clinicians as needed.
  • Interdepartmental Communication: Communicate effectively with various departments, including Marketing, Provider Affairs, and SSD, to ensure seamless delivery of healthcare services.
  • Confidentiality and Compliance: Maintain complete confidentiality of company business and comply with HIPAA, Diversity Principles, Corporate Integrity, and other applicable policies.

Requirements
  • Education: Bachelor of Social Work or Psychology, or LVN, LPN with 1 year of experience in managed care systems, or RN, or 3 years of care coordination experience in a state-managed or waiver program, or 3 years of managed care systems experience.
  • Knowledge and Skills: Medical terminology, experience coordinating member medical needs, providing assistance to members, and analyzing member needs, PC proficiency, customer service skills, verbal and written communication skills, and coaching skills.
  • Additional Requirements: Current state driver's license, transportation, and applicable insurance, ability and willingness to travel, and previous behavioral health experience.
