Role Summary:

The Resident Service Coordinator plays a pivotal role in enhancing the quality of life for residents in affordable housing. This position is dedicated to providing education, referrals, and essential services to residents, particularly those who are frail or at risk. By empowering individuals to maintain their independence, the Coordinator ensures that residents can exercise their Right to Self Determination.

Key Responsibilities:

  • Conduct thorough, non-clinical assessments to evaluate residents' wellness and social needs.
  • Assist residents in identifying, accessing, and coordinating necessary services, including personal care.
  • Monitor the delivery of services and maintain communication with service providers to ensure resident engagement.
  • Develop and implement educational and preventative health programs tailored for residents.
  • Foster partnerships with local agencies, community service providers, and stakeholders to enhance service delivery.
  • Maintain an updated directory of local resources and service providers.
  • Exhibit professionalism and excellent communication skills within the property management team.

Daily Expectations:

  • Manage daily online time card entries and documentation of resident case management.
  • Build and maintain strong collaborative relationships with property management.
  • Participate in monthly departmental meetings and training sessions.
  • Understand and navigate local, state, and federal benefits and entitlements.
  • Adhere to training requirements and quality assurance guidelines.
  • Travel locally for community networking and attend relevant conferences.

Qualifications:

A bachelor's degree in a social service-related field or equivalent experience is required. Candidates should possess knowledge of elderly and disability services, referral processes, and local resources. Experience working with elderly populations or individuals with disabilities is preferred. Bilingual skills and training in cultural competency are advantageous.

Skills and Abilities:

  • Strong communication skills, both written and verbal.
  • Ability to perform basic mathematical calculations.
  • Critical thinking and problem-solving capabilities.
  • Physical ability to perform job functions, including lifting up to 25 pounds.
