About the Position

The Intensive Case Manager (ICM) will provide assistance and aid individuals and families requiring assistance from LifeSTEPS. The ICM will work under the direction of the Project Manager to promote and live out the LifeSTEPS workplace values of integrity, innovation, empowerment, respect, and personal growth in tandem with the Oz Principle.

Core Duties

The ICM will be responsible for:
  • Assessing participants to identify needs such as personal and family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments to determine the nature and degree of assistance.
  • Promoting, coordinating, and tailoring services to meet identified needs.
  • Must be physically present at each assigned property as scheduled and spend the required contracted hours at each assigned property per month.
  • Assisting participants individually, with family, or in other small groups regarding plans for meeting needs; aiding participants to mobilize inner capacities and environmental resources to improve social and economic functioning.
  • Advocating and referring participants to community resources, human services, and other organizations as needed.
  • Identifying the need for and assisting in the development of auxiliary services to facilitate bringing applicants into job-ready status.
  • Instructing applicants in resume writing, job search, and interviewing techniques.
  • Compiling records and preparing reports.
  • Completing all administrative assignments, including timely data entry in multiple databases.
  • Reviewing service plans and performing follow-ups to determine the quantity and quality of services provided to participants and the status of participant cases.
  • Accessing and recording participant and community resource information.
  • Securing supplementary information such as employment, medical records, or school reports as needed.
  • Studying and assessing the strengths and weaknesses of existing resources.

Job Requirements

The ideal candidate will have a Bachelor's degree in social work, psychology, or a related field, and two years of experience in a similar environment providing social services. A valid California driving license and reliable, insured transportation are required.
