Key Responsibilities- Provide Crisis Intervention: Offer immediate support to homeless veterans and their families, addressing their critical needs and connecting them with essential resources.
- Assess and Develop Housing Goals: Conduct thorough assessments to identify individuals' strengths, needs, and preferences, and work with them to create personalized housing plans.
- Implement Housing Stability Plans: Design and execute comprehensive plans to help individuals achieve stable housing, including connecting them with community resources and services.
- Monitor Progress and Document Services: Regularly track individuals' progress, maintain accurate records, and ensure compliance with program regulations.
- Enforce Program Rules and Procedures: Uphold all government and contract regulations, ensuring that our programs operate effectively and efficiently.
- Provide Access to Services and Resources: Facilitate connections between individuals and community resources, promoting self-sufficiency and stability.
- Facilitate Housing Transitions: Assist individuals in transitioning to transitional and/or permanent, independent housing, when appropriate.
- Conduct Home Visits: Visit individuals in their homes, as needed, to provide support and ensure their well-being.
- Determine Eligibility: Assess individuals' eligibility for our programs and services.
- Process Temporary Financial Assistance: Assist individuals in accessing temporary financial assistance, as needed.
- Utilize Data Management Systems: Effectively use data management systems, such as HSMIS, to collect and record case information.
- Education and Experience: A minimum of a bachelor's degree in social work or a related field, with a minimum of two years' experience serving homeless or at-risk families and/or individuals in crisis.
- Knowledge and Skills: Possess basic computer skills, excellent interpersonal skills, and the ability to interact effectively with diverse populations. Must be sensitive to and respect cultural diversity, and able to work with individuals from various racial, ethnic, and economic backgrounds.
- Physical and Mental Demands: Must be able to work in a fast-paced environment, handle new and diverse work problems, and make effective decisions under pressure. Must be able to relate and interact with volunteers, clients, contractors, visitors, and employees at all levels within the Agency.
Our work environment is dynamic and fast-paced, with occasional exposure to noise, communicable diseases, and verbal abuse. As a member of our team, you will be expected to work outside of the established work schedule or work odd hours, as needed. All information associated with the Agency is confidential.
