Case Manager

Job Summary:As a Case Manager, you will provide mental health, substance abuse, and outreach services to individuals with mental health and/or substance use disorders. You will work with community partners and local stakeholders to provide comprehensive care, ensuring continuity of care with clients through case management services and linking clients and their families to needed resources to improve their quality of life.
Responsibilities:- Provide individual and group mental health and substance use disorder case management services
- Refer, advocate, and link clients and their families for additional services as identified
- Provide supportive and educational counseling services to develop daily living and coping skills
- Assist clients with developing natural support systems
- Transport clients in company vehicles to necessary appointments as needed
- Develop, implement, evaluate, and update treatment plans with input from the client and family
- Conduct clinical behavioral health assessments
- Enter written documentation into Electronic Medical Record in a timely manner for all services rendered
- Act as liaison and communicate collaboratively with local law enforcement and local stakeholders to maintain coordination of the treatment process
- Complete all required agency trainings to maintain 100% compliance
- Maintain departmental productivity expectations
- Bachelor's degree required
- Good organizational, time management, written, and verbal communication skills
- Ability to develop supportive relationships with persons with mental health and substance use disorders while maintaining appropriate therapeutic boundaries
- Community-based program case managers must have a valid FL driver's license
- Knowledge of community resources or ability to learn quickly
- Experience working with individuals with mental health or substance use concerns
