About the Role

As a Case Management Director, you will be responsible for leading a team of case managers and overseeing the day-to-day operations of our Case Management department. You will work closely with our interdisciplinary team to develop and implement effective care plans that meet the unique needs of our patients. Your expertise and leadership skills will be essential in driving patient outcomes and ensuring the highest level of quality care.

Key Responsibilities
  • Lead a team of case managers and oversee the day-to-day operations of the Case Management department
  • Develop and implement effective care plans that meet the unique needs of patients
  • Collaborate with the interdisciplinary team to ensure seamless care transitions and optimal patient outcomes
  • Provide guidance and support to case managers and other staff members
  • Build relationships with insurance companies, self-insured employers, and other healthcare networks
Requirements
  • Current CCM or ACMTM certification preferred
  • Must be qualified to independently complete an assessment within the scope of practice of his/her discipline
  • Must hold an active license if licensure is required for the discipline within the hospital's state
  • Three years of hospital-based Case Management experience, including Utilization Review and Discharge Planning experience
