Physical Therapist-Homecare-FT

We are seeking a skilled Physical Therapist to join our Homecare team. As a Physical Therapist, you will be responsible for providing professional rehabilitation therapy to patients in their homes, evaluating their condition, and developing individualized treatment programs to help them achieve their goals.
Responsibilities- Evaluate patients in their homes to assess their condition and develop a comprehensive treatment plan.
- Implement individualized treatment programs to help patients achieve their goals and improve their functional abilities.
- Document patient progress and communicate with other healthcare team members to ensure effective coordination of care.
- Act as a liaison between physicians, medical directors, and other healthcare team members to ensure seamless care coordination.
- Supervise and educate patients and caregivers on proper techniques and exercises to promote optimal recovery and independence.
- Participate in discharge planning and ensure a smooth transition of care for patients.
- Bachelor's Degree in Physical Therapy (PT)
- Minimum of six (6) months of PT experience
- Physical Therapist License
- AHA Basic Health Care Life Support HCP Certification
