Responsibilities
As a Speech Therapist, you will be responsible for delivering professional therapy services to patients in their homes. This includes evaluating, developing, and implementing individualized therapy programs in accordance with organizational policies and external standards.
- Conduct comprehensive patient evaluations within designated timeframes, ensuring a holistic assessment of the client's condition.
- Create tailored treatment plans based on evaluation results, focusing on functional, measurable, and time-oriented goals.
- Prepare electronic medical record documentation, including progress notes, reassessments, and discharge summaries, ensuring compliance with policies and procedures.
- Maintain precise, neat, and legible documentation that supports the necessity for continued skilled care in the home setting.
- Ensure service delivery aligns with the frequency and duration outlined in the physician's plan of care.
- Act as a liaison among physicians, managed care providers, and other healthcare team members to facilitate effective coordination and resource referral.
- Educate patients and/or caregivers on a written home program, submitting a copy for the medical record as per policy.
- Provide comprehensive education tailored to the identified needs of patients and/or caregivers in an understandable manner.
- Build rapport and mutual respect with patients to create an optimal environment for achieving goals, utilizing appropriate clinical skills and techniques.
- Participate in discharge planning throughout the patient's care journey, addressing any issues that arise.
- Complete scheduling requirements accurately and promptly, updating and submitting new or changed schedules through the appropriate system.
- Respond to voicemails and messages in a timely manner.
- Document communications with other healthcare team members throughout the patient's care using secure messaging systems.
- Ensure discharge summaries accurately reflect the patient's current status, outcomes achieved, and recommendations for further services.
- Perform additional duties and projects as assigned, demonstrating flexibility in job responsibilities.
- Adhere to HMH's organizational competencies and standards of behavior.
- Meet physical demands, including lifting a minimum of 50 lbs., pushing and pulling a minimum of 50 lbs., and standing for a minimum of 8 hours a day.
Qualifications
Education, Knowledge, Skills, and Abilities Required:
- Master's Degree in Speech Pathology.
- At least six (6) months of experience in Speech Pathology.
- Proficient computer skills.
Licenses and Certifications Required:
- Speech Language Pathology License.
- Basic Life Support (BLS) certification.
