Responsibilities
The Speech Therapist is responsible for delivering professional therapy services to patients in their homes. This includes the evaluation, development, and implementation of personalized, goal-oriented therapy programs in line with organizational policies and external regulations.
- Conduct thorough patient evaluations within the designated timeframe, ensuring a comprehensive assessment of the client's condition.
- Create individualized treatment plans based on evaluation results, focusing on functional and measurable short- and long-term goals.
- Document progress notes, reassessments, and discharge summaries in the EMR system, ensuring compliance with policies and reflecting skilled care.
- Maintain precise, neat, and legible documentation, submitted promptly to support the need for continued skilled care.
- Ensure service delivery aligns with the frequency and duration specified in the physician's plan of care.
- Act as a liaison among the physician, MCP, and other healthcare team members to facilitate effective coordination and resource referral.
- Educate patients and/or caregivers on home programs, providing written instructions and submitting copies for medical records.
- Deliver comprehensive education tailored to the identified needs of patients and/or caregivers in an understandable manner.
- Build rapport and mutual respect with patients to create an optimal environment for achieving therapy goals, employing appropriate clinical skills and techniques.
- Engage in discharge planning throughout the patient's care, addressing any issues as they arise.
- Meet scheduling requirements accurately and timely, updating and submitting changes through the appropriate systems.
- Respond to communications promptly and document interactions with other healthcare team members securely.
- Ensure discharge summaries accurately reflect the patient's status, outcomes, and recommendations for further services.
- Perform additional duties and projects as assigned, demonstrating flexibility in job responsibilities.
- Adhere to HMH's organizational competencies and standards of behavior.
- Meet physical demands, including lifting a minimum of 50 lbs., pushing and pulling a minimum of 50 lbs., and standing for a minimum of 8 hours a day.
Qualifications
Education, Knowledge, Skills, and Abilities Required:
- Master's Degree in Speech Pathology.
- At least six (6) months of experience in Speech Pathology.
- Proficient computer skills.
Licenses and Certifications Required:
- Valid Speech Language Pathology License.
- Basic Life Support (BLS) certification.
