Overview:
The Clinical Nurse Navigator plays a crucial role in reviewing patient care to foster effective collaboration with Radiologists, Physicians, and Medical Consultants.

This position operates with minimal oversight, acting as a vital resource and communication hub for both clinical and administrative interactions, utilizing expertise to develop, assess, and prepare essential operational and clinical protocols.

Guidance from senior management is sought for complex cases.

Responsibilities:
Conducts thorough reviews of patient cases, medical histories, and current treatment plans to adeptly guide patients through their healthcare experience.


Creates comprehensive care plans in partnership with various healthcare providers and departments, ensuring a seamless and supportive journey for patients.

Maintains detailed records of patient interactions, care plan developments, and navigational interventions to guarantee consistent and coordinated care.


Utilizes knowledge of healthcare systems, resources, and patient requirements to assist patients throughout their medical journey, ensuring access to necessary services and support.

Evaluates navigational strategies and interventions to ensure they prioritize patient needs.


Engages in consistent communication with healthcare teams and patients, ensuring clarity and continuity of care, while also serving as a key resource for the organization in enhancing patient navigation services, occasionally seeking guidance from senior management for intricate navigational challenges.

Performs additional duties as assigned.

Qualifications:

EDUCATION:

Required:
Associate's Degree

Preferred:
Bachelor's Degree

EXPERIENCE:

Required:
5 years as a Registered Nurse and 1 year in a clinical specialty as recognized by ANCC or national nursing organizations.

LICENSURE/CERTIFICATION/REGISTRY/LISTING:

Required:
Registered Nurse license in North Carolina or a Compact state

BLS (CPR)-American Red Cross or AHA Healthcare Provider
