Job Description

Catholic Charities Family Community Services is seeking a highly skilled and compassionate Care Coordination Program Manager to join our team.

The ideal candidate will have a strong background in care management, case work, and supervisory experience. They will be responsible for providing administrative oversight and ongoing support to the Program Supervisor in the Traumatic Brain Injury (TBI) Waiver and Nursing Home Transition and Diversion (NHTD) Waiver Service Coordination programs.

We offer a competitive salary of $27.00 - $27.00 Hourly, depending on experience, and a comprehensive benefits package.

In this role, you will:

  • Provide support to supervisor and staff including but not limited to, hiring, training, and ongoing staff development.
  • Complete administrative tasks as assigned including but not limited to Housing, UAS, etc.
  • Promote and encourage team building within the Care Coordination department and throughout the agency.
