Salary Range:The estimated salary for this role is between $46,800 and $53,600 per year, or approximately $23-$26 per hour, based on a 40-hour workweek.
Job Description:Job Summary: We are seeking a skilled Health Home Care Manager to join our Care Management team. This role requires outreach and engagement with potentially eligible patients, obtaining consents, and coordinating care plans.
Key Responsibilities:
- Screen patients for Health Home functional scale eligibility and conduct initial assessments and reassessments of patient needs.
- Collaborate with medical providers and patients to develop, implement, and coordinate Health Homes compliant care plans.
- Provide direct service to a caseload of chronically ill patients, facilitating referrals, addressing access issues, and connecting them to local resources.
- Coordinate patient services and care with pharmacies, insurance companies, hospital discharge planning, family caregivers, and other providers.
- Provide crisis intervention when needed and maintain patient confidentiality at all times.
Ideal Candidate: The ideal candidate will possess strong working knowledge of local community resources, demonstrated ability to work with data reporting, documentation, and outcomes, and experience navigating multiple data management systems.
