We are seeking a dedicated Care Coordinator for Dementia Patients who will be part of our interdisciplinary team. The successful candidate will provide comprehensive care coordination services to maintain a long-term care partnership for people living with dementia and their caregivers.

The Care Coordinator will deliver care consultations, assist with behavioral management, medication management, caregiver burden, and situations involving abuse or neglect. They will also provide required training for care navigators upon hire and annually, build and deliver a dementia care navigation training program, and support and coordinate all care needs including access to clinical and community support services for GUIDE beneficiaries.

A key responsibility of the role is to execute quality of life and home safety assessment screening tools to beneficiaries to assess home safety, behavior, function, medication monitoring & support needs, care needs, and advanced care plans. The Care Coordinator will also develop personalized care plans based on the desires and needs of each beneficiary and their unpaid caregivers.

In addition, the role requires coordinating and participating in interdisciplinary care communication and conferences, reviewing and updating care plans as the participant's condition evolves, and completing required written documentation in the electronic health record and other required reporting systems on a timely basis.

To be successful in this role, you must have a strong work ethic, excellent communication skills, and the ability to organize your day and work independently while being flexible and working in a team environment. You should also be able to identify and implement best practices to enhance quality of life for people living with dementia and their caregivers.

This position requires extensive travel, and the ideal candidate will have a valid New York State Driver's License and reliable transportation. We offer a competitive salary of $25.00 per hour and a supportive work environment that aligns with our company values.
