Job Description

We are seeking a highly skilled Patient Care Facilitator to join our team at First Choice Community Healthcare INC. The successful candidate will be responsible for coordinating care for patients, facilitating access to community resources, and participating in quality improvement initiatives.

About the Position

The Healthcare Facilitator will work closely with healthcare delivery teams to coordinate care, conduct patient chart reviews, and implement healthcare quality improvement activities. This is an excellent opportunity to make a meaningful difference in the lives of our patients and communities.

Responsibilities
  • Coordinate patient care, including pre-visit planning and post-visit follow-up
  • Facilitate access to community resources and services
  • Participate in quality improvement initiatives to enhance patient outcomes
  • Document patient interactions and progress in electronic medical records
Requirements
  • Medical assistant training or comparable clinical experience
  • Direct work experience in a healthcare setting, preferably ambulatory
  • Knowledge of medical terminology and patient intake procedures
  • Bi-lingual English/Spanish highly preferred
