This position requires strong analytical and problem-solving skills, as well as the ability to work effectively with clients, community stakeholders, and interdisciplinary care teams.

The ideal candidate will have experience working with social services, behavioral health, or healthcare settings, and be able to complete a NM Community Health Worker Certification within the first year.

Responsibilities include:

  • Assessing client needs and identifying resources
  • Advocating for systems change with community stakeholders
  • Collaborating with clinical providers to facilitate care coordination
